Rise Training Program Application Form

Thank you for your interest in participating in one of Rise's Entrepreneurship Training Programs, we're excited to meet you! We offer several training programs ranging from 8 to 10 weeks. All of our programs are offered online in virtual classrooms.

All of our programs offer the following benefits:

• Business development: Gain the skills and confidence to develop your idea and launch your business.
• Networking and coaching: Network with like-minded individuals and build connections with local entrepreneurs and experienced business professionals.
• Financing: Once you graduate, you may be eligible for a start-up grant or to apply for low-interest small business loan from Rise.
• Mental health considerations: Factor in your wellness and build a mental health plan in your business development.
   

Candidates Qualifications

Applicants for our programs should have all the following characteristics:

• Have a practical idea to start or grow your own for-profit, small business (any size, either full or part time/seasonal opportunities are welcome), and be looking to start this within the next 6 months.
• Commit to attend all course sessions and submit all assignments.
• Self-identify as having experienced mental health or addictions challenges.
• Be over the the age of 16.
• Have access to a computer (smartphones and tablets are not recommended) and reliable internet to attend classes and complete course work.

Once we have reviewed your application, we will reach out to let you know which program you are eligible for, the next cohort dates and times and answer any questions you may have. We are really looking forward to getting to know you and to supporting you as you take steps to bring your business to life!

Rise is committed to inclusivity and to creating a learning environment that is respectful to all. You may come across some gender terms that you are unfamiliar with below. We are happy to provide clarification as to the terms:

• Cisgender (Cis Woman or Cis Man): a person whose gender identity corresponds with the sex the person had or was identified as having at birth
• Transgender (Trans Woman or Trans Man): a person's gender identity that’s different from the gender or sex assigned at birth
• Nonbinary: Individuals who are nonbinary can experience gender a variety of ways, including a combination of man and woman, neither man nor woman, or something else altogether.
• Third gender: Originating in non-Western and Indigenous cultures, third gender is a gender category that includes people who have a gender that can’t be exclusively categorized as man or woman, or is different from man or woman.

We will need to collect personal information from you to process your application and for administrative purposes. The information we collect may include your name, birth date, e-mail address, phone number, home address, demographic data, financial status, your business idea and business plan. We will use your personal information for the following purposes:

Process your submitted application;
Build relationships, facilitate networking opportunities and match entrepreneurs like you with mentors;
Assist with administrative or technical support of your participation in the program;
Gather your opinion and feedback through surveys;
Comply with Canadian legal and regulatory requirements;
Maintain statistical and record keeping operations.
 

Your personal information may be collected, used or disclosed for any of the identified proposes set out above. We will not use or disclose it for other purposes without obtaining additional consent from you. Please note that your consent is necessary for us to process your application and for administrative purposes if you are accepted. You may withdraw your consent at any time upon reasonable written notice, subject to legal or contractual restrictions. Please note that the withdrawal of consent will affect our ability to provide you with the products and services that you would like to receive.

To achieve the identified purposes listed above, your personal information may be shared with third parties such as government or industry agencies, mentors and advisors to your business. Sometimes you may be working with a referral agency to help you prepare your business plan and start up your business. In this case, your personal information may be shared with this third party, based on your consent.

Canada’s Anti Spam Law:

From time to time Rise Asset Development may wish to send you emails as part of the administration during your program or provide advice to your business. We also send out group e-mails providing information such as program updates, event information or other information of interest to our clients.

I authorize Rise Asset Development to:

• Contact referral agency or agencies for the identified purposes above
• Use my e-mail address to administer my application and if approved, my participation in the program
• Keep my name on an email distribution list to receive information

By signing below you are authorizing Rise Asset Development to collect, use, or disclose your personal information that you provide to us for the identified purposes listed above.
