Scholarship Assistance: Group

This application form is for use by Church/Group Leaders requesting scholarship assistance for one or more campers in their group. Campers attending camp independently of a group, should use the Individual Scholarship – Youth application.

Please submit this Group Scholarship Application a minimum of 4 weeks in advance of your scheduled camp dates. Be prepared to list the specific campers in your group who are in need of Forest Home Scholarships.

To request a Scholarship, please list each individual camper (one line per camper) and include a brief description of the circumstances surrounding their financial need (e.g. Single parent home, major illness, unemployment, homelessness). For the FH Registration Cost, include only the Forest Home registration fee, not any additional costs your church or group may add. For Church Scholarship Amount, indicate what your church is contributing financially to assist this camper. List additional campers in the comments section. Please do not submit separate applications for each camper.
