As adult and/or Parent/Guardian, I consent for myself or my child to attend this event. I agree to hold harmless HEALING GROVE HEALTH CENTER INC., A SOCIAL PURPOSE CORPORATION and HEALING GROVE HEALTH CENTER FOUNDATION INC, THE CITY OF SAN JOSE, its employees, staff, volunteers, members, agents, assigns, including but not limited to risks associated with this event, any other type of activities and from any and all liability associated with this event including but not limited to any and all medical decisions on my behalf by the supervising adults and any and all injuries incurred by myself or my children at this event. This paragraph shall be construed broadly to allow supervising adults to make any and all emergency medical decisions on my behalf until I am able to do so.
Electronic Checkin at this event constitutes an electronic signature, and agreement to the following Waiver and Hold Harmless Agreement.
