    2. **Unauthorised Use of Restraint or Seclusion**: Incidents involving the use of physical, mechanical, or chemical restraint or the seclusion of a participant without proper authorisation or in a non-compliant manner.

    3. **Serious Injury**: Incidents resulting in serious injury to participants. This includes both accidents and injuries that result from the provision of support or lack thereof.

    4. **Medication Errors and Management**: Incidents related to the incorrect administration or management of medication that could harm or adversely affect the participant.

    5. **Death**: Any death of a participant, regardless of cause, while under the care of a service provider.

    6. **Service Access or Delivery Issues**: Issues related to the denial of service, inappropriate service delivery, or lack of access to necessary services that adversely affect the participant's well-being.

    7. **Participant Rights Violations**: Incidents that involve the violation of participants' rights, including privacy breaches, discrimination, and lack of respect for dignity and choice.

    8. **Quality of Support Concerns**: Concerns regarding the quality of support provided, including issues with the competence of staff, adequacy of care plans, and overall service quality.

    9. **Unexplained Absences**: Situations where a participant is unexpectedly absent, missing, or unaccounted for.

    10. **Health and Safety Concerns**: Incidents that pose a risk to the health and safety of participants, including environmental hazards, inadequate infection control, and lack of safety measures.

    11. **Complaints Management**: Issues related to the handling of complaints by the provider, including delays in response, lack of resolution, or unsatisfactory outcomes.

    12. **Financial Mismanagement**: Incidents involving the mismanagement of a participant’s funds or financial exploitation.

    For each category, it's important to gather detailed information about the incident, including the date, time, individuals involved, and a description of the event. This will help in accurately reporting to the NDIS Commission and ensuring that appropriate actions are taken to address and prevent future occurrences.
