IMPORTANT

Proof of payments may consist of copies of the following documents:

- Double-sided digital copy of cashed check

- Copy of credit card statement

- Copy of bank statement clearly showing payment by direct/electronic deposit or Interac payment

Receipts issued by service providers are not accepted and no refunds will be made for payments made in cash.

By signing this form, you certify that your municipal accounts are in good standing and that your service providers have been paid in full for payments related to this claim.
