Please send us this duly completed form at the end of your 3rd or 6th month of operation (depending on your letter of offer) so that we can send you the 2nd and final installment of the contribution that was previously approved.

IMPORTANT
Proof of payments may consist of copies of the following documents:
- Double-sided digital copy of cashed check
- Copy of credit card statement
- Copy of bank statement clearly showing payment by direct/electronic deposit or Interac payment
Receipts issued by service providers are not accepted and no refunds will be made for payments made in cash.
