By signing below I acknowledge understanding that;
- A discharge procedure is the process when a patient's case is discussed in a treatment team format, which includes, but is not limited to, medical staff, counselors, office management and support staff. Administrative discharge may be immediately entertained by a staff member who is threatened or abused in ANY WAY.
- It is the policy of A New Crossroad to discharge patients from all services when a certain condition is met. The following list includes but is not limited to conditions of discharge
- Repeated failure to show up for appointments
- Failure to keep your accounts current
- Failure to follow program expectaions, rules, and/or guidelines
- Acting out in the office, IE: disruptive behavior in the office, on the property, by phone or by electronic means
- Threatening staff members or other patients with or without reason
- Loitering in the facility or on the property without reason
- Joint agreement between the patient and the counselor when treatment has been completed
- Buying, selling, trading of drugs or other contraband on the premises of A New Crossroad
- Selling or giving away (diversion) of any medication written by a provider at A New Crossroad.
If a patient is discharged by A New Crossroad by the administration we will supply a 30 day prescription for all medication to allow the patient to find a new provider.
If a patient fails to come in for appointments or self discharges, A New Crossroad is under no obligation to provide any further medications.
