2024 Annual Golf Tournament Registration Form

Reservations are due NO LATER THAN June 9th, 2024
This years golf outing will be held at Conklin Players Club. This years golf package
includes golf, lunch, dinner and awards program. There will be a grilled lunch at
the turn and a shotgun start at 10:00 AM. A chicken BBQ dinner will be held at
3:00 PM in the clubhouse with the awards program. The cost is $135.00 for an all
day package. If you do not golf you can attend the dinner for only $30.00.
