Manager - Name and email (all volunteer credits unless shared, then 2 credits)
Co-Manager (if applicable) - Name and email (2 volunteer credits)
Treasurer - Name and email (2 volunteer credits)
Head Coach - Name and email (all volunteer credits unless shared, then 2 credits)
Assistant Coaches - (List all) Name and email (2 volunteer credits)
Goalie Coach - Name and email (1 volunteer credits)
Trainer - Name and email (no credits)
Parent Liaison - Name and email (1 volunteer credit)
Team Raffle Coordinator - Name and email (2 volunteer credits)
Jersey Parent - Name and email (1 credit per jersey set)
Volunteer Coordinator (1 volunteer credit)
Tournament Committee Member (U7/U11/U13 only) - Name and email (2 volunteer credits)
The administration will use this information for the season. Anyone who is not listed will not be considered Team Staff and therefore will not be able to par-take in any Appreciation Events or collect their volunteer credits if allotted. If something should change, the team Manager is required to advise the administration asap.
