Instructions

Fill out the online application form and proceed with submission.
Print the form. (A PDF download link will be provided upon submission, as shown below.)
Ensure the application is signed, and affix a passport photo of the applicant in the designated space.
Present the duly signed application accompanied by the appropriate membership fee. to the club during office hours - on Fridays from 17:00 to 18:00.

N.B - In case of applicants under the 14years of age, this form must be filled in and signed by one of the parents or legal guardian/s, who also need to write their full name as well as their ID card no, in the spaces indicated below (Refer to the section - Legal Guardian Consent).
