Instructions

Complete and print the application form (a PDF download link will be provided at the end).
Ensure the application is signed and attach two passport-sized photographs of the applicant in the designated spaces provided.
Submit the duly completed and signed application form, together with the applicable membership fee, to the club during official office hours.

N.B - In case of applicants under the 14years of age, this form must be filled in and signed by one of the parents or legal guardian/s, who also need to write their full name as well as their ID card no, in the spaces indicated below (Refer to the section - Legal Guardian Consent).
