Complete this form to enroll your student for the Before and After School Program with Bow Recreation Center.  This registration allows ONE student per submission.

Read the policy booklet and attachments in the original email before committing to this program.  The policies outlined in the booklet are enforced, and by completing this registration, you agree to abide by those policies.

POLICY AGREEMENT

On behalf of myself, student, and family, we received, read, and understand the Before and After School Program Policies revised April 2023 as well as all other documents provided. 

By continuing the enrollment process, we agree to abide by these policies.

Additional Pickup and/or Emergency Contacts

This next section allows up to three additional Pickup/Emergency contacts.  Do not repeat anyone entered previously in this registration.

Also note, this record will replace any current record for your student.  If you do not list someone who was on last year's record, they will be removed.  These contacts do not carry over from a previous file.

The following items are highlights from the policy booklet and are not considered a substitute for reading the booklet.  Please take the time to read the policy booklet.

Schedule Commitment

I understand the weekly schedule submitted involves a commitment for the school year and tution is due whether or not my student is in attendance (including school closures, holidays, vacation weeks, etc).

I understand that a two week notice is required for schedule changes.

If I withdraw my student, a two week notice is required with payment due whether my student attends or not.

If I withdraw my student and later want to re-enroll them, the new schedule will be based on availability.

Drop-In Policy

I understand I can contact the Bow Rec Center to schedule one-time Drop-Ins for days that are not part of my regular schedule; however, drop-Ins are approved if the schedule allows additional students for that day.

If you pay tuition via manual check, payment for a Drop-In is due before or on the Drop-In day to avoid late fees.

If you pay tuition via Tuition Express, payment will process when the Drop-In is scheduled.    

School Closures

I understand I will have the opportunity to register for additional hours to accommodate teacher workshops, inclement weather, school vacation weeks, etc. 

These are situations where school is closed, but the Bow Recreation Center is open.

If the Bow Recreation Center is closed, there will be no additional hours offered.  Refer to the holiday schedule attached to the original email.

Additional tuition is due for the additional hours upon registration.  Refer to the fee schedule attached to the original email for details.

If you register your student for additional hours, but decide not to attend, the fees are not refundable.    

Reporting Absences

In addition to contacting the school, I will contact Bow Recreation Center via phone or email by 11 am if my student will be absent from their regularly scheduled day.  There is no refund for absences.  

Holidays

I understand that the Bow Recreation Center is closed for major holidays and that tuition is due if these days fall on my student's regularly scheduled days.

Please refer to the holiday schedule attached in the original email. 

Licensing

I understand that municipal recreation programs, including the Before and After School Programs and Summer Recreation Programs, are exempt from requiring licensure and that the Tow of Bow Parks and Recreation Before and After School Programs and Summer Recreation Programs are not licensed by the NH Department of Health and Human Services.  

Emergency Evacuation Plans

In the event we need to evacuate Bow Recreation Center (BRC) due to a confined emergency, the staff and children will leave the building and gather in the immediate area at Bow Elementary School (BES), 22 Bow Center Rd, Bow, NH  03304.  The BES contact is Lori Krueger, Principal, 603-225-3049.

In the event we need to evacuate BRC due to an emergency in the immediate area, the staff and children will walk or be transported by Bow School Bus to the Bow Community Center (BCC), 2 Knox Road, Bow, NH  03304.  The BCC contact is Malinda Blakey or Michele Myrdek at 603-223-3920.  

If necessary, children will be transported to Concord Hospital, 250 Pleasant St, Concord, NH  03301.  Contact number for Concord Hospital is 603-225-2711.

The Primary, Secondary, and Emergency contacts identified in this enrollment will be contacted in that order until someone is reached.  

Tuition Express Payment Authorization

The Bow Recreation Center/Town of Bow uses Tuition Express (TE), which is an automated payment processing system that allows on-time tuition and fee payments such as registration fees, snack/water fees, additional hours enrollment fees, etc. for the Before/After School Program. 

Tuition Express Late Fees and NSF

Tuition Express (TE) -  program fees process every Monday.  Tuition is due for the enrolled schedule (including holidays, vacation weeks, and weather related closures) whether or not your student is in attendance.

A $25 service fee will be charged for non-sufficient funds or failed ACH transactions.
