MINOR PARTICIPATION RELEASE
By submitting this registration form, I hereby certify that I am the adult parent or guardian of PARTICIPANT, a minor child under the age of eighteen years, and I consent to their participation in summer 2023 classes at Spokane Civic Theatre. I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of said minor child’s participation in performing arts training. I recognize my responsibility to ensure that said minor child participates only in those activities for which they have the required skills, qualifications, training, and physical conditioning. To the fullest extent allowed by law, I hold harmless and agree to indemnify Spokane Civic Theatre, its officers, directors, faculty, staff, volunteers, employees, and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in performing arts training at Spokane Civic Theatre, resulting from any cause whatsoever, and regardless of fault.
Spokane Civic Theatre has fee and royalty free permission to use PARTICIPANT’S likeness in photo/videography for print and online marketing/promotional purposes associated with Spokane Civic Theatre only for perpetuity.
PARTICIPANT will not be under the influence of drugs or alcohol while on Spokane Civic Theatre property or participating in Spokane Civic Theatre associated activity offsite.
Spokane Civic Theatre is committed to a harassment-free environment for its staff, volunteers, patrons, contractors, students, and guests and consequently enforces a zero-tolerance harassment policy:
Spokane Civic Theatre, Inc. prohibits the unlawful electronic, psychological, racial, religious, sexual, and/or workplace harassment on Spokane Civic Theatre property (1020 North Howard Street, Spokane, WA 99201) or the harassment of a Spokane Civic Theatre staff member, volunteer, patron, contractor, student, or guest at any off-site, Company associated or affiliated event, function or other informal meeting of any kind. Any and all harassment-based concerns should be immediately reported to Executive Director: 509-325-2507, jake@spokanecivictheatre.com. Spokane Civic Theatre shall pursue all necessary steps to ensure any harassment-based concern(s) is promptly investigated and addressed (legally or otherwise at the discretion of the Company).
