By submitting this application. we authorize North Rock Rentals Ltd. to make inquiries to the banking & business references we have supplied. Customer agrees to pay all invoices Net 30 days from date of invoice. Invoices unpaid after thirty days after invoice date will be considered past due and continuation of credit terms will be at the discretion of North Rock Rentals Ltd.
Proof of Insurance must be supplied prior to any rental.

To include:

1) Insured: name, address, policy number & expiry date

2) General liability: minimum $1,000,000 per occurrence bodily & property damage 3) Replacement value of the equipment rental

4) North Rock Rentals Ltd to be added as "additionally insured" under
general liability policy and "loss payee" under the rental equipment coverage policy

5) 30 days notice of cancellation or altercation of all such listed above

NSF Cheques are subject to a $45.00 fee.

Customer certifies that all information provided on credit application form is true. In return for such extension of credit, customer hereby agrees to all foregoing terms and conditions, intending to be legally bound hereby.

Credit cannot be granted without a signature acknowledging credit terms.
