LIABILITY WAIVER*

I hereby certify that my child(ren) is/are in good physical condition and do/does not suffer from any disability that prevents or limits his/her participation in all activities conducted by STS Academy. I acknowledge that STS will not assume any responsibility or liability for personal injury or damages caused by any injury. In the event STS staff is unable to reach a parent, guardain or any emergency contact. I hereby give permission for my child(ren to be transported to the nearest hospital for treatment in case of an accident or emergency. I hereby further authorize(s) any of the staff or employees to provide for, approval and authorize health care to hospital.

During your child(ren)'s attendance in the summer program he/she may participate in an activity that is being photographed or videotaped; these photographs/video recording may be used for promotionsl purposes. If you do not agree to your child(ren) likness being recorded you must make the request in writting.

Cancelation & Refund

We understand plans change. We will gladly issue a full refund for any cancellation requests received more than 7 days before the start of the program. No refunds on cancellations notices received less than 7 days before the first day of the program.
