AUTHORIZATION TO SEEK MEDICAL ATTENTION ‼️‼️. I, the undersigned parent or legal guardian of the above-named participant, authorize the coaches, instructors, staff members, volunteers, and authorized representatives of Royalty D.N.A. Dance Empire to seek and obtain emergency medical treatment for my child if I cannot be reached immediately and if such treatment is deemed necessary due to illness, injury, or other medical emergency occurring during practices, performances, competitions, travel, or any event sponsored by Royalty D.N.A. Dance Empire.I understand that every reasonable effort will be made to contact me before medical treatment is administered. However, if I cannot be reached, I authorize licensed physicians, emergency medical personnel, hospitals, and other healthcare providers to provide treatment they determine to be necessary for my child’s health and well-being.I understand that I am financially responsible for any medical expenses incurred as a result of such treatment.This authorization shall remain in effect until revoked in writing or until the end of the current dance season, whichever occurs first. TUITION FEE & POLICY

At Royalty D.N.A. Dance Empire, we are committed to providing quality dance training while building discipline, confidence, responsibility, and excellence in every dancer. Tuition payments help support instruction, studio operations, choreography, and the overall dance experience.

Registration Fee
A $25 non-refundable registration fee is due at the time of registration to secure your dancer’s enrollment.
Registration is required before participation in classes begins.

Monthly Tuition
Monthly tuition is:
$50 per dancer per month
Tuition payments are due on the 15th of each month.
Tuition reserves your dancer’s placement in class and is based on enrollment, not attendance. Monthly tuition remains due regardless of absences, missed classes, vacations, or schedule conflicts.

Sister Discount
Royalty D.N.A. Dance Empire offers a family discount for sisters enrolled in the program:
2 Sisters Monthly Tuition Rate: $85 total per month
This discounted rate applies to enrolled sisters only and must remain active to continue receiving the discount.

Payment Policy
• Tuition must be paid by the monthly due date of the 15th. • Payments received after the due date may be subject to additional fees or restrictions. • Dancers with outstanding balances may be unable to participate in classes, performances, or events until accounts are brought current. • All payments are the responsibility of the parent/guardian listed on the registration form. • Tuition payments are non-refundable and non-transferable.

LIABILITY WAIVER

Release and Waiver of Liability

In consideration for being permitted to participate in activities with Royalty D.N.A. Dance Empire, I, the undersigned participant and/or parent/legal guardian, hereby release, waive, discharge, and hold harmless Royalty D.N.A. Dance Empire, its owners, directors, instructors, staff members, volunteers, affiliates, and representatives from any and all claims, liabilities, damages, injuries, or expenses arising from or related to participation in dance activities, except where prohibited by law.

This release applies to injuries or damages that may occur during classes, rehearsals, performances, competitions, travel, events, or any activity associated with Royalty D.N.A. Dance Empire.

ASSUMPTION OF RISK WAIVER

I understand that participation in dance classes, rehearsals, performances, competitions, workshops, and related activities involves physical movement and certain risks, including but not limited to falls, strains, sprains, muscle injuries, collisions, and other accidental injuries.
I acknowledge that dance is a physically demanding activity and that participation requires proper instruction, preparation, and personal responsibility. I voluntarily choose to participate and assume all risks associated with dance activities provided by Royalty D.N.A. Dance Empire.

Medical Responsibility

I understand that I am responsible for ensuring that the participant is physically capable of participating in dance activities. I agree to notify Royalty D.N.A. Dance Empire of any medical conditions, injuries, limitations, or concerns that may affect participation.
I acknowledge responsibility for any medical expenses, treatment, or emergency care required as a result of participation.

Photography & Media Release

I grant permission for Royalty D.N.A. Dance Empire to use photographs, videos, or other media featuring the participant for promotional, educational, and organizational purposes, including social media, website content, marketing materials, and performances.
