Please complete the sports physical form prior to checking in (form is also found on the LS Activities Website page or in the Business and Activities Office). Please note, parent must sign ahead of time. No refunds will be issued. $10 of each physical goes directly back to the designated group.

Please do not drink sports/energy drinks before physical. Space is limited and check-in closes at 6:45 pm.
