I voluntarily give permission for my child to participate in Circle of Friends program from the date listed below until their high school graduation, or if I rescind their participation.
I will help my child maintain regular contact with his/her mentor(s), meeting a few times each month throughout the year.
I will work in cooperation with the mentor(s) to arrange meeting times and activities.
I agree to give the mentor(s) a working telephone number where I can be reached and two emergency contacts in case I am not available.
I will respect the mentor(s) and keep confidential any information given me regarding them or their family.
I understand that the mentor is NOT a childcare provider.
I will provide program related information to program staff and participate in evaluation activities.
I understand that if my child engages in illegal activates towards Circle of Friends or Circle of Friends staff, volunteers and mentors, Circle of Friends may require the child/youth to connect with a Restorative Justice Counselor and the child's/youth's mentor will have the option to leave the relationship.
I understand that no one will make plans with my child or take my child without my permission and that the mentor(s) will provide me with a contact telephone number where they can be reached.
I will notify program staff of any concerns, questions or problems related to my child, and/or the mentor(s) of the program. (i.e. health, relationships, care, housing, custody, education).
I understand that program staff is required by law to report suspected instances of child abuse or neglect to the Department of Human Service, Child Welfare Division.
I will respect the beliefs, values, and dignity of all people.
As the parent/guardian of a child participating in Circle of Friends, I agree to the above
