EnerSys Video/Photograph Release

I hereby grant EnerSys and its affiliates and subsidiaries (“EnerSys”) the irrevocable right and permission to use photographs or video recordings of me taken at EnerSys’ facilities on its websites, publications, derivative works, or for any other similar purpose without compensation to me, including such use in training or marketing efforts.

I understand and agree that such photos or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such pictures, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of EnerSys.

I hereby release, acquit and forever discharge EnerSys and its successors, assigns, affiliates, officers, directors, shareholders, employees, agents, and representatives from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

SPARK Museum Terms and Conditions

Media Release

I hereby grant the SPARK Museum permission to take photographs or video of my child named on this form for future use in publicity and marketing materials such as print, digital, television, or social media. Materials may be used by SPARK Museum and partners for publicity and marketing. I understand that my child will not be identified by name in any future promotion. By signing this form, I hereby certify that I am the parent and/or guardian of named minor on this form or have the authority to grant this permission.

 

Medical and Liability Release

I authorize the SPARK Museum and its staff to obtain emergency medical treatment for my child in the event of a life-threatening emergency. My signature authorizes my child to be treated by the first available medical facility and physician should the need arise and authorizes my emergency contact listed above to pick up my child from the program and make decisions regarding my child and their care if I am not available. I understand that every effort will be made to contact me in the event that such an emergency should take place.

In order for my child to participate in eventswith the SPARK Museum, I hereby waive, release, and discharge any and all claims for damages for personal injury, and property damages or which may hereafter occur to me or my child as a result of participation in said event. This release is intended to discharge in advance the SPARK Museum, its officials, officers, employees, volunteers, and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.
