ID Requirements for Minors

We are excited to provide a service for you or your family member. Please carefully read this form to help us avoid the frustrating situation of not being able to do so. Failure to bring proper IDs mean that we will not be able to provide services, which is disappointing for everyone involved. Most importantly, for the young person getting a piercing!
Minors must be accompanied by either a parent or legal guardian.

Please no aunts, uncles, siblings, or grandparents as the only accompanying adult. Proper IDs and documentation must be presented for both the young person and for the parent/guardian. The parent/guardian must be present throughout the duration of the service. Please no drop-offs!

Two original forms of ID are required. Photocopies and phone pictures are not acceptable.

Bring a BIRTH CERTIFICATE* along with one of the following:

• A valid state or government issued photo ID such as a driver’s license, permit, ID card or passport.

• A valid school photo ID.

• Most recent yearbook.

*BIRTH CERTIFICATES must be certified or original. We cannot accept copies, most of which are stamped with VOID.

One form of ID is required. Bring a valid state or government issued ID such as a driver's license, permit, ID card, military ID or passport.
