Accident Waiver and Release of Liability Form
As the event host and organizer, I hereby assume all risks associated with my participation and the participation of my guests in any and all activities related to this event. This assumption of risk includes, without limitation, risks arising from negligence, carelessness, or the use of equipment or property owned, maintained, or controlled by my guests or me. I accept full responsibility and liability, regardless of fault, for any death, disability, personal injury, property damage, property loss, theft, or other actions or claims arising from or related to the event.
I represent and warrant that I, and all guests attending under my reservation, are at least eighteen (18) years of age and are not under the influence of alcohol, drugs, or any other substances that would impair judgment, safety awareness, or the ability to act responsibly.
I acknowledge and understand that, during the event, my guests and I may be photographed or recorded. I grant permission for such photographs and/or video recordings to be used for any lawful purpose deemed appropriate by the authorizing entity, including but not limited to marketing and promotional activities for Erotic Toy Chest, LLC.
I understand and agree that all payments are non-refundable. I may reschedule my party booking one (1) time within ninety (90) days of the original event date. If the event is not rescheduled within the ninety (90) day period, all payments shall be forfeited.
I acknowledge that this Accident Waiver and Release of Liability Form will be relied upon by the event organizers and shall govern our conduct, rights, and responsibilities in connection with the event. This Waiver and Release shall be interpreted and enforced to the fullest extent permitted by applicable law.
