Accident Waiver and Release of Liability Form

As the party planner attending and hosting this event, I hereby assume all of the risks of my guests, and myself, participating in any/all activities associated with this event. This includes any risks that may arise from negligence or carelessness or from dangerous equipment or property owned, maintained, or controlled by my guests and me. I assume full liability without fault of the death, disability, personal injury of my guests and me, and any property damage, property theft, or actions of any kind.

I, as well as all of my guests attending will be over the age of 18 years of age, and is not under the influence of drugs or alcohol which impairs my ability to maintain my safety awareness or endangers others.

I understand while participating in the activities, my guests and I may be photographed. I agree to allow our photos and/or videos to be used for any legitimate purpose this authorizing entity decides, such as marketing and promotions with Erotic Toy Chest, LLC.

I understand that payment is non-refundable, however I am allowed up to 90 days to reschedule my party booking. If the party booking is not rescheduled within 90 days, I forfeit my payment.

I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of the activities in which we may participate, and that it will govern our actions and responsibilities as members attending this event. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
INVOICE FOR PAYMENT WILL BE SENT TO THE EMAIL YOU PROVIDED. PLEASE COMPLETE THE INVOICE AS SOON AS POSSIBLE TO RESERVE YOUR PARTY BOOKING DATE AND TIME.
