Payment Details

The break down of enrollment fee is listed below:

Book                                            $80

National Phlebotomy Exam.   $200

Total                                           $700

 Only 5 Students Per Class Session a deposit of $200 and completion of this form is required to hold your place.

Refund Policy

If you decide to cancel your enrollment within a week of school, then we will be providing a full refund minus registration fee.

If it is 8-15 calendar days, then only half will be refund.

If it is more than 15 calendar days, we will not be providing any refund, which the student is highly advised to attend classes.

 

Attendance Policy

The Student should be present at all classes except if he/she is sick or has an emergency. All work will be made up on their own time and I will send what needs to be studied.

If the student has 2 or more absences the student will have to pay a $100 fee for a makeup session (This is a requirement and student can not take exam until session is made up)

Academic Performance

The student is required to have an average grade of 85% for class overall and 85% on each test and National Phlebotomy Exam.

Behavior

The student should always practice good manners and professional conduct. Students who violate will receive a verbal warning and may end up removed from program. (Without any form of refund)
