PMH-C Documentation Submission

Please submit all documents as a PDF. Incomplete documents cannot be processed.

Upload the following documentation as part of your PMH-C application:

1. Certificate from one approved 14-hour PMAD training

2. Certificate from 6 hours of advanced training

3. Employment Verification/Private Practice Attestation: Please do not submit paystubs. You may submit more than one employment verification form if you have multiple roles that indicate you meet the 2 years of post-graduate employment in the profession. The forms must be signed using the certified digital signature or a handwritten signature. A typed signature cannot be accepted.

4. Verification of your Educational Degree/Certificate(s): This could include unofficial transcripts, a copy of your diploma. For the Affiliated Professionals Track, this could include a copy of any certificate(s) earned as well. Please note: Verification of a professional license does not meet this requirement.

Advanced Training*

I completed a PSI Advanced training (Advanced Psychotherapy, Advanced Psychopharmacology).I completed another PSI training that qualifies for Step 2 (Perinatal Loss, Foundations in Paternal Perinatal Mental Health, Perinatal Substance Use Disorders).I completed an alternate Step 2 training.

If you completed an alternate Step 2 training, please check all of the following that best describe the course that was completed.*

The training was 6+ hours.The training was delivered as a live/interactive course.The training covers an advanced topic related to perinatal mental health for my track (Psychotherapy, Psychopharmacology, Affiliated Professionals).N/A - I completed a Step 2 training from PSI.

Submit your Employment Verification/Private Practice Attestation that verifies 2 years of post-graduate employment in the profession. *
