    TUTORING PROGRAM PARENTAL CONSENT FORM AND RELEASE AGREEMENT

    This form is required to ensure that all tutoring and coaching provided by the tutors volunteering for the Silent Victims of Crime (SVC) Tutoring Program is conducted with the full knowledge and approval of the parents/guardians whose children are receiving services from tutors. This form also grants the tutor and SVC Tutoring Program permission to discuss and share information about the student only with the purpose of best supporting the student's learning.

  • Location of sessions: Zoom Sessions, previously coordinated and approved by SVC Tutoring Program.

    I, as parent/guardian of the above-named student, request that a tutor volunteering for the Tutoring Program of Silent Victims of Crime be allowed to tutor/coach my child after school on the days and times previously agreed between myself and SVC Tutoring Program Coordinator. I am aware that SVC tutors may be high school students (maybe under 18 years old) with high academic performance and/or an adult with subject area knowledge being part of the SVC Tutoring Program as a volunteer. The tutoring session will be virtual, through the Zoom platform. These sessions may be recorded and supervised by Silent Victims of Crime to follow up on the evolution of the sessions and the Program.

    I understand that any communication between me, my child, and the tutor may occur only by email, copying the SVC Tutoring Program email (tutoring@silentvictimsofcrime.org), or through the Silent Victims of Crime Tutoring Coordinator and/or SVC Programs Administrator.

    I understand that I am responsible for the supervision of my child during any authorized virtual tutoring session.

    Iunderstand that I am responsible for monitoring my child's progress in tutoring sessions and school performance.

    I hereby release Silent Victims of Crime, its Board members, Directors, employees, and volunteers from any and all liability of whatever nature or kind arising from receiving tutoring services virtually, including all risks connected therewith, whether foreseen or unforeseen.
