This form will just gather some essential information from you that will allow us to make your experience amazing. Once you get to the end, you'll see instructions on how to book your first appointment with us. We can't wait to meet you!

  • Covid-19 Procedures, Health, and Safety:
    To protect the health of our staff we ask that you ensure you are in accordance with the following guidelines before booking an appointment. If you have an appointment reserved and do not meet these guidelines we ask that you reschedule your appointment to avoid a cancellation fee. Please do not book an appointment if you are not in accordance with the following;
    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
    I further acknowledge that Hunter Donia has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and all other communicable diseases/infections.
    I further acknowledge that Hunter Donia can not guarantee that I will not become infected with the Coronavirus/Covid-19 or any other communicable disease/infection.
    I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any other disease/infection may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
    I voluntarily seek services provided by Hunter Donia and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.

    I attest that:
    * I am not experiencing any symptoms of illness such as cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
    * I have not traveled internationally within the last 14 days.
    * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
    * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
    * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

    For your comfort and safety, please notify your stylists if you have allergies, any physical issues or disabilities, or if you are pregnant. If you experience discomfort or any other symptoms during the treatment, please alert your service provider immediately.

  • Cancellation Policy:
    We understand that things can come up. We value your time, and hope you value ours too. In the event you need to cancel your appointment, please be sure to notify us no later than 48 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee. To ensure our cancellation policy is upheld we require a credit card to book any appointment. Any appointment not cancelled within 48 hours of the scheduled date and time falls under our cancellation policy protocol. You will be charged a fee depending on how long of an appoinment you had booked. This fee must be paid in order to re-book. If you have any questions regarding this policy please communicate them before committing to an appointment. Fees range from $25-$100..

    Saved Payment Method:
    All guests' payment methods are safely and securely stored on file using my POS system. My POS system uses Stripe, the world’s leading secure and PCI compliant online payment processor.

    Service Adjustment Policy:
    Your satisfaction is the highest priority. If you are not completely content, please let us know during your service or within 7 days after your service has been performed. We are committed to making any needed corrections, as long as it falls within the services discussed and agreed upon during the consultation. 

    Product Return Policy:
    A lightly used product can be exchanged for another product by your next scheduled appointment. Please reach out as soon as possible to let us know your issue with the product.

     

    Confidentiality and Privacy:
    Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies.

    Kids at appointments: 
    We aim to provide a laid back, luxury experience for each guest. We want you, as well as other guests in the salon to be able to relax & enjoy their visit. The salon may also be harmful to children due to hot tools, chemicals, and sharp objects. The salon can be a very busy place and though we love your little ones, we ask that you not bring them to your appointment. If you need to reschedule your appointment, we are happy to help.     Once you click submit, you will be redirected to the online booking page. If the redirect does not work, check your email for the direct booking link!

     

    Booking Instructions:

     

    Step 1: Once you're finished here head back to Step Two on our website New Guest Page.

     

    Step 2: Find the "NEW CLIENT Appointment" option.

     

    Step 3: Follow the prompts to book your appointment.

    You will be required to put a Credit Card on file (stored securely) to book, however you will not be charged anything until the end of your first appointment with us, in which you can use whichever payment method most convenient for you.

     

    If you're unable to book an appointment that works well for you, please add yourself to our waitlist. This helps us to keep track of guest who would like to be seen. We can often further assisst you. You can also specify dates and times that work for you and will be automatically notified if an availability opens.
