Thank you for choosing Rex's Animal Rescue to help choose your new best friend! It is very important to fill out this application in as much detail as possible. Please remain truthful in your answers, as negligence can cause a pet to be placed in an innappropriate home causing unnecessary stress on us, you and the pet. All current pets in the home must be up to date on vaccines, heartworm prevention, and spayed or neutered.

Each pet in our care is spayed/neutered, vaccinated and heartworm tested. There will be a mandatory spay/neuter form and proof of spay or neuter for any puppy or kitten under 5 months of age will be expected and asked for at any time. If adopter fails to provide proof, all adoption terms will be eliminated. EVERY PET ADOPTED MUST BE SPAYED/NEUTERED. THERE WILL BE NO EXCEPTIONS.

By signing this form you agree to not only send photos of your fence and yard, but to the possiblity of having a volunteer come out to your home to evaluate if it is suitable for the pet you would like to adopt.

Adoption fees for puppies/dogs 4 years old and younger is $300, adult dogs 5 + is $200. Kittens 1 year old and younger is $150, adult cats is $100, and senior cats are $75. All adoption fees are non-refundable after contract is signed. Please note if you are adopting outside of the North Texas area, it is very likely there will be a transport fee that must be covered by the adopter or, if Rex's Animal Rescue is able, by donations.

Please understand that we are all volunteers with full time jobs. It could take 3-4 days to process an application. We reserve the right to not approve someone for adoption. Please e-mail us with inquiries about a specific pet you would like to adopt.

Please understand that pets are a 15+ year commitment! So many pets are returned for someone no longer having time for them, so please be sure you consider what the forseeable future holds. If you're concerned that you may not have the time to spend with your new pet PLEASE DO NOT ADOPT!
