LIABILITY WAIVER AND RELEASE FORM

In consideration of being allowed to participate in the10U Spartans Baseball tryouts, I, [Signee below], hereby agree to the following:

1. I understand that baseball tryouts involve physical activity that may be hazardous and that I am voluntarily participating in these activities with knowledge of the risks involved.

2. I acknowledge that there are risks associated with baseball tryouts, including but not limited to, physical injury, illness, and death, and I assume full responsibility for any such risks.

3. I hereby release and hold harmless [name of organization], its officers, directors, employees, volunteers, and agents, from any and all liability, claims, demands, and causes of action that may arise from my participation in the baseball tryouts.

4. I agree to indemnify and hold harmless CFL Spartans Baseball, Inc, its officers, directors, employees, volunteers, and agents, from any and all liability, claims, demands, and causes of action that may arise from my actions during the baseball tryouts.

5. I understand that this waiver and release form covers any and all activities associated with the baseball tryouts, including transportation to and from the tryouts.

6. I acknowledge that I have read this waiver and release form, fully understand its terms, and sign it voluntarily without coercion.
