    Welcome to Lingfield Prep! We're so excited to have your child here.

    Could we please ask that you complete this ‘All about me’ booklet to the best of your knowledge, this will help inform us about your child’s needs and provide some early information that will aid your child’s transition.

    We look forward to meeting you soon and discussing your child and their transition at more length during their induction and subsequent settling-in sessions.

    Please note that we will contact your child’s current setting for information to help support your child during the transitional settling in period.
  • Has your child attended any other Early Year’s setting prior to starting here?*
  • EYFS Statutory Guidance

    providers must record the following information for each child in their care
  • Does your child live with both parents?*
  • Does your child have any special educational needs/additional needs/disabilities?*
  • Do they/have they had an Individual Education Plan (IEP) or an Educational Health Care Plan (EHCP)?*
  • Medical information

    The following information may be needed in the event of an emergency. Please bring your child’s ‘Red Book’ with you when you come in for your first settling in session.
  • Is your child up to date with their vaccinations?*
  • Does your child need medication administered on a daily basis?*
  • Please note that in an emergency, our staff may be asked to provide information to NHS staff regarding your child's medical history. I give permission for staff at Lingfield College to seek any necessary emergency medical advice or treatment and to discuss my child's medical history if needed*
  • Information Sharing

    It may sometimes be helpful to share information about your child's development with other agencies/settings, particularly at the time of transition. You will always be informed when this is happening and who with.

    The Characteristics of Effective learning provides us with an understanding of how your child learns and how they apply it during self-directed play and engagement; for example: what characteristics do they demonstrate independently when making choices or showing curiosity. Please tick the statements which apply to your child.
