In addition to the essay, please attach the following documentation:
You will find that this application is similar to the Free Application for Federal Student Aid (FAFSA). As you may know, the FAFSA is a standard application that is used to apply for federal government student aid (grants and loans) and other aid supplied by other entities.
You may wish to assemble all of your information, complete your FAFSA, and then complete this form, as it may then be easier to complete this application.
To complete both the FAFSA and this application, you may need the following documents:
This application for the 2024-2025 academic year will request information from the 2022 tax year.
You will find that some of the FAFSA questions are not asked, and those question numbers are marked as being intentionally omitted.
Notes for the corresponding FAFSA questions are provided at the bottom of each page.
If you need assistance completing this form, please email us at OLGS@uchbg.org, or telephone 717-564-4761 to leave a message on how we may contact you to be of assistance.
The numbers in these questions closely correlate to the July 1, 2024 - June 30, 2025 FAFSA form. If you complete your FAFSA first, you will have most of the information you will need to complete this form. You will notice some questions are omitted intentionally.
Notes to FAFSA questions have been provided below.
Understand that references made to IRS Forms are for the year 2022.
If the answer is “No” to both of the questions above, and the student is not married, questions 20–22 can be skipped; however, if the student is also required to provide parent information on the form, question 22 must be answered.
Convert all currency to U.S. dollars. If the answer is zero or the question does not apply, enter 0.
If the answer is “No” to both of the questions above, question 28 can be skipped.
Terms and Conditions
The LaGrone Scholar will be eligible to receive the scholarship funds subject to the terms and conditions. If these terms and conditions are not met, or if the Scholar is found to have falsified or altered any school record or other information submitted to the Oliver LaGrone Scholarship Committee, the Scholar will forfeit any funds that have not been disbursed.
The Oliver LaGrone Scholarship is offered only to graduates of Harrisburg High School, including John Harris and SciTech campuses, Cougar Academy and Harrisburg Virtual Learning Academy.
The scholarship is open to applicants to an accredited post-secondary institution such as a college, business school, technical school, school of nursing, or an institute of fine arts.
To receive the scholarship money, the LaGrone Scholar must provide the following items to the Scholarship Committee by July 1st and January 1st of each year:
The Scholar will maintain communication with their LaGrone Mentor at least three times a year, July to June. It is recommended that the Scholar and Mentor keep a record of when contact is made.
If the Scholar fails to communicate with their Mentor or with the Scholarship Committee for a period in excess of 365 days, the Scholar will forfeit their eligibility to receive any additional disbursements from the Scholarship Fund.
The Scholar will notify the Scholarship Committee and/or their Mentor of post-secondary and home addresses, email and telephone numbers during their school career. Scholarship checks are mailed to the most recent address provided to the Scholarship Committee.
The Scholar may specify whether the check is to be mailed to their home address or their post-secondary address. Scholarship checks will be made payable to the Scholar, or, if the Scholar so designates, to the Scholar and the institution of higher learning.
If the Scholar is not registered for any post-secondary classes or scheduled for any formal lessons or formal assignments within their training curriculum for 730 consecutive days, they will forfeit any additional disbursements from the Scholarship Fund. The Scholar may appeal to the Scholarship Committee but the Committee is not required to reinstate their eligibility to receive undisbursed funds.
Parent and student agree that by signing this application, you certify that all of the information you provided is true and complete to the best of your knowledge.
