Informed Consent and Acknowledgement
I, hereby grant approval for my child to participate in the Ice Flame FC soccer summer camp. I understand that this involves physical activity, and I assume all risks and hazards incidental to the conduct of the activities, including transportation to and from the activities.
I acknowledge that the Ice Flame FC soccer summer camp, its staff, coaches, and volunteers will not be responsible for any injury or illness incurred during participation in the activities. In the event of any injury or illness, I hereby authorize the camp staff to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my child's immediate care.
I release, indemnify, and hold harmless Ice Flame FC soccer summer camp, its officers, directors, agents, coaches, and employees, from and against any and all claims, demands, damages, actions, suits, or proceedings arising from or related to any loss, damage, or injury (including death) to my child or any other person or property caused by, arising out of, or in connection with my child's participation in the Ice Flame FC soccer summer camp.
I also acknowledge that my child is physically able to participate in the camp activities, and I assume all risks associated with participation in the activities, including but not limited to, bodily injury, illness, property damage, or loss of personal property.
I have read this informed consent and acknowledgement form, fully understand its terms, and sign it freely and voluntarily without inducement.
