Suggested donations are collected on first day of class, cash or check only please. After completing this form you will receive a confirmation email with class and location details as well as a membership form to complete your registration. By signing below you acknowledge that (1) you would like to enroll your child(ren) in the selected summer enrichment class, (2) you agree to notify us as soon as possible if you need to cancel your enrollment, (3) deposits are not refundable, and (4) if you are not already a current member of Meadow View Learning, you will need to sign the Membership Agreement form which will be emailed to you in order to complete the registration process.
