Lab Field Trip Request Form

Thank you for your interest in organizing a field trip at the Cade Museum for Creativity & Invention. We strive to provide a unique and motivating experience for both students and teachers. Please note that the completion of this request form does not guarantee your reservation. Upon receiving your request, we will review it with our program department and you will be contacted to confirm the details of the trip. Your reservation will be secured when the Field Trip Contract is signed and deposit is received.
All field trips must be booked, with a signed contract, at least 2 weeks prior to the date of the field trip.

Please review the capacity limits and pricing for lab field trips:

Lab & Explore- 80 student maximum, 20 student minimum, $15 per student
Grade Level
Number of Students (age 5-17)
10 adult chaperones are included at no additional charge for each field trip reservation. Any additional adults will not be included in the field trip, and will be required to purchase museum general admission. Only the designated 10 chaperones will be admitted into the lab portion of the field trip experience.
I understand and acknowledge that only 10 included chaperones will be admitted to the lab spaces.

Lab & Explore field trips may take place Wednesday, Thursday, and Friday from 10am-12pm.

Topics may be changed up to 1 week in advance of your field trip. Our Cade Educator will answer any topic or activity related questions during your phone call 1 week prior to your trip.
