    Youth Program Application

  • Thank you for your interest in our youth programs! Please complete the following application in its entirety. Information provided will only be used for the purposes of this program and shared only with members of the Clarksville Alumnae Chapter of Delta Sigma Theta that are volunteers.  The programs below are completely free to the community as long as each student meets the requirments below.

    • Delta Academy (Middle School Girls ages 11-14)
    • Delta GEMS (High School Girls ages 14-18)

    By completing this application, I and my parent/guardian acknowledge the following:

    1. I reside in CAC's service area (Clarksville-Montgomery, TN, Robertson County, TN, or Fort Campbell, KY)
    2. I will attend monthly sessions between September and May 
    3. I must complete independent learning activities and/or a community service projects

    Forms can also be signed during the parent orientation.
