I am voluntarily participating in the "Maranatha Fellowship’s “Youth & Young adult gathering" (also stated as "Activity") on 4th Sunday's. I and any participants I am responsible for will participate in the Activity entirely at my own risk. I am aware of the risks associated with participating in this Activity, especially as it relates to COVID-19 and its effects, which may include but are not limited to: physical injury, pain, suffering, illness, or death. I understand that these injuries or outcomes may arise from my own or other's negligence, or from conditions at the Activity location. Nonetheless, I assume all related risks, both known and unknown to me and any participants I am responsible for at this Activity. I further acknowledge that I will not hold Maranatha Fellowship Church or any of its authorized persons responsible for any ordinary negligence, and give up or waive any right that I otherwise have to bring legal action against MARANATHA FELLOWSHIP CHURCH for personal injury or property damage.
