We want to share records with the conventional practice of your choice
1. For those going to Audubon Family Veterinary Center ask for your $25 Pathways to Wellness Discount
Please go to https://www.audubonfamilyvets.com/pathways to let them know you are coming or to request a visit. They will want to upload your records to their software system before your visit. When you arrive, tell them you are one of our clients as they are offering you a $25 discount on your first visit.

Please note this is ONLY for primary care services. Ace of Spays is still fully functioning for all spay/neuter care and Pathways To Wellness will remain accessible for Chiropractic, Chinese Herbal and Acupuncture care.
2. For those going to Other Conventional Practices

Please complete this form to have your Pathways To Wellness records shared with your preferred veterinary clinic. Audubon Family Vet will e-mail them for you. Please note this is ONLY for conventional care services. Ace of Spays remains fully open for all spay/neuter needs and Pathways To Wellness will remain accessible for Chiropractic, Chinese Herbal and Acupuncture care.
