    I hereby acknowledge and grant permission to Future Leaders of Tomorrow Academy to use photographs or videos of my child, taken during their time at the school.

     

    I understand that these photographs or videos may be used for various school-related purposes, including but not limited to, school publications, promotional materials, various social media platforms, and the school's website.

     

    I release Future Leaders of Tomorrow Academy from any liability or claims arising out of the use of the photographs or videos.

     

    I understand that this permission is ongoing and will remain in effect.

  • Attendance Policy/Agreement

    Future Leaders of Tomorrow Academy believes that regular school attendance is essential for academic success. To ensure your child's success, we ask that you read and agree to the following attendance policy:

     

    • Attendance is mandatory. Students must arrive no later than 9:00 AM and the school day begins at 8:30 AM.
    • Chronic absences and tardiness may result in suspension or termination from the school. Students who miss 10 or more days of school without a valid excuse will be considered chronically absent.
    • Excused absences must be supported by prior notice or a doctor's note. Excused absences include illness, doctor appointments, or family emergencies. If your child is going to miss school, please notify the school office in advance.
    • Unexcused absences include but are not limited to truancy, personal reasons, and vacations.

     

    This policy constitutes a contractual agreement between Future Leaders of Tomorrow Academy and the student's parent or legal guardian. Failure to comply with this policy may result in disciplinary action.

     

    By signing below, I acknowledge that I have read and agree to the attendance policy of Future Leaders of Tomorrow Academy. I understand that regular attendance is essential for my child's academic success, and I will make every effort to ensure my child's attendance and punctuality.

  • Mandatory Parent Nights

    At Future Leaders of Tomorrow Academy, we believe that parents and staff work together as partners to ensure all scholars succeed in all areas of their lives. To solidify this partnership, we have established the following parent night policy:

     

    • Parent nights will be held once a month. Parents/guardians must attend each parent night (in-person or online)
    • Parent night attendance is mandatory 

     

    This policy constitutes a contractual agreement between Future Leaders of Tomorrow Academy and the student's parent or legal guardian. Failure to comply with this policy may result in student suspension.

     

    By signing below, I acknowledge that I have read and agree to the parent night policy of Future Leaders of Tomorrow Academy. I understand that a parent is required to attend each parent night.

  • Uniforms & Materials Policy/Agreement

    At Future Leaders of Tomorrow Academy, we believe that school uniforms create a sense of unity and belonging among our students. To ensure consistency and a professional learning environment, we have established the following uniform and materials policy:

     

    • Uniforms are mandatory for all students. Students must come to school wearing a solid polo shirt with the FLOTA logo and khaki, navy blue, or black bottoms. All skirts/dresses must be worn with shorts. Dresses must also have the school's logo and must be knee length.
    • Students must wear closed-toe shoes with their uniform every day.
    • Students must come to school with a book bag daily, their FLOTA folder, and other assigned materials.
    • Clothing and accessories that may cause a disruption or distraction to the learning environment are not allowed. Examples include but are not limited to hats, bandanas, excessively baggy clothing, and distracting jewelry.

     

    This policy constitutes a contractual agreement between Future Leaders of Tomorrow Academy and the student's parent or legal guardian. Failure to comply with this policy may result in disciplinary action.

     

    By signing below, I acknowledge that I have read and agree to the uniform and materials policy of Future Leaders of Tomorrow Academy. I understand that my child is required to wear a uniform and come to school prepared with the required materials.
