How did you hear about our program?
Did one of our current families refer you? We currently provide a family reward program for referrals.
Is your child age 3-12? We do not enroll children less than age 3. We provide two different programs that are our Private Preschool for ages 3-4 and School age childcare for ages 5-12. If your child is not 3 they would not be able to be enrolled.

Can your child function in a classroom group setting of approx 20 children. We are unable to do 1 on 1 groups at our facility.

Our hours of operation are 7am-5:30pm. Are you able to drop off and pick up during our regular business hours as this is a contractual requirement?

Does your child currently have an IEP? We will need a copy of it if so. We work very closely with the school districts to help all children.

Does your child have therapy appointments include speech, occupational, or other? We do allow therapy visits at daycare and have several therapists that come to our facility weekly and need this information for room scheduling. This is available to you if you need it.

If your child is ages 3-12 and you have an already approved case we can do a change of provider and CCP will approve. If you do not have a current case you will be enrolled as private pay only until approval letter is issued. Do you understand this process? We do not accept DCFS payments only Community Child Care Connections.

I understand payment is due every Friday PRIOR to the week of care for private pay parents. There are no exceptions to this. Any payments not received on Fridays will not be able to drop off on Mondays until brought current. $25.00 daily late fees are applied to daily rate including weekend dates for any payments not received on time. Some parents chose to pay monthly or biweekly according to their work pay dates. This is absolutely fine so long as paid prior to week of care. Do you understand this contractual requirement & are you able to be complaint with this to attend?
