Course Agreement

My goal is to provide quality care to all my studets in a timely manner. No-shows, late arrivals, and last minute cancellations cause an inconvenience not only for me, but for other clients wanting to book an appointment. I have VERY strict appointment policy in place. To reserve a spot in my schedule for you, I require that you agree to my Course Agreement/Deposit Form, as follows:

Regristering

A non-refundable deposit of $500 is required at the time of regristering your training course and will go towards your tuition being scheduled. The deposit secures the date and time.

The remaining balance is due the day of your training date by debit, credit, afterpay or cash.

Note: AFTERPAY is subject to 5% taxes & a service fee of 5%

Cancellation and Refund Policy

Once you set your Training date that you chosen above, you will get an email if those dates are approved. If denied, an email will be sent, and you can select alternating dates that would be available. Once approved, your 1:1 Training is secured. You may be able to move a different date one time only, with at least 14 days notice. Anything less then 14 days notice, you will forfeit your refund, and a new deposit would have to be sent again to secure a new date.

Please be advised that deposits for enrollment are NON-TRANSFERABLE and NON-REFUNDABLE. No exception

After filling out your course agreement, you will receive an email within 2-48 hours if your preferred date is approved or denied.

It is the responsibility of the student to ensure they put the correct date and time in their calendars once their spot is confirmed - my booking system will send out reminders to the email provided at booking but I do not send out personal reminders.
