Conditions of attendance - Coronavirus If performers are invited to attend an audition or a casting session, they must advise the Agency and casting director if they have, in the last 14 days before the audition, travelled to areas deemed at highrisk from Coronavirus by the NHS.

(Please find the list of countries here: https://www.nhs.uk/conditions/coronavirus-covid-19/ This also applies to any performers who believe they may have Coronavirus or are showing symptoms of it, as well as any performers who have been in close contact with someone with Coronavirus. Performers that fit within those categories above should self-tape rather than attend auditions in person. Details of how the agency, casting director and production company who are responsible for managing the production will accept self-taped auditions will be available on request. Auditions will be assessed equally and fairly regardless of whether the audition is self-taped, or made in person.

Should a performer be successfully cast for and accept a role in the production, they should continue to follow all NHS/PHE advice on how to avoid catching or spreading Coronavirus, follow FCO travel advice and take all precautions to avoid risk. If a performer believes they will be unable to attend the shoot (whether for health reasons, restrictions in travel or other reasons), they should inform the Agency and casting director as soon as possible.

They agree to keep fully confidential all information received relating to the casting, including details of advertisers, the advertiser's product, the intended commercial and any shoot

They are also agreeing that if successfully cast for this role, they will produce a medical certificate if it should prove necessary. advising on their fitness or otherwise to work on this production.

The information you provide on this form will only be used for the purposes of this casting process. If you are unsuccessful, it will only be retained for as long as we deem reasonably necessary for our record keeping purposes. If you are successful, it may be kept for as long as the ad is used. It may be shared with those involved in the ad's production, such as the client/advertiser, agency, casting agent, your agent, production company and the insurers of the ad. In certain cases, you may request access to and the correction or erasing of your information or the restriction of its processing.

Your information will only be processed where there is a legal basis for doing so. Generally, it will be used to help decide whether to enter into a contract with you to perform in the ad, orfor legitimate interests such as evaluating your performance at the casting, or based on your consent (for example with regard to any sensitive information such as your racial or ethnic origin, or your health
