  • FOSTER/ADOPTION AGREEMENT

  • And I, hereafter referred as FOSTER/ADOPTER agrees to the following terms and provisions as listed below: 

    1. FOSTER/ADOPTER agrees to indemnify and hold harmless GRSF, its members and representatives against any and all claims, known or unknown, now or hereafter arising in connection with this dog.
    2. FOSTER/ADOPTER consents to interview and/or examination of this dog by GRSF at any time and acknowledges that GRSF has the right to reclaim the dog at any time for failure to comply with the terms of this contract and/or non-payment of required adoption donation.
    3. FOSTER/ADOPTER understands that if the FOSTER/ADOPTER cannot keep the dog, the FOSTER/ADOPTER must return the dog to GRSF. The dog shall not be given away, sold, or exchanged without prior written consent from GRSF. In the event of the Fosters’/Adopters’ death, GRSF should be notified.
    4. FOSTER/ADOPTER understands that if for any reason this dog has not been spayed or neutered prior to the execution of the contract, the FOSTER/ADOPTER agrees to have the appropriate surgical procedure performed and provide documentation that this procedure has been performed. FOSTER/ADOPTER acknowledges that until this has been accomplished, his/her adoption of the dog is not complete and that GRSF has the right to and will reclaim the dog.
    5. FOSTER/ADOPTER agrees to pay GRSF for any and all expenses, including court costs and reasonable attorney’s fees, in enforcing the terms and provisions of this contract. Payment of these damages will not exclude the FOSTER/ADOPTER from returning the dog to GRSF.
    6. FOSTER/ADOPTER agrees that GRSF is no longer financially responsible for any costs incurred as of the date of an adoption.  I have read the available history of the above-mentioned dog and understand that the dog is adopted as a family pet companion. GRSF makes no warranties or representation regarding the dog’s health, medical status, disposition, behavior, or temperament. It is agreed and understood by the FOSTER/ADOPTER that every effort is made to provide an accurate history and assessment of the dog. It is agreed and understood by the FOSTER/ADOPTER and GRSF that this written agreement sets forth all promises, agreements, conditions and understanding between them, orally or written, that both parties have fully read and understands all of this agreement. The signature of the adopting party or parties below operates as consent to relieve GRSF and its volunteers of any liability and/or obligation of conduct to the adopting party or parties as related to the above statements.
    7. FOSTER/ADOPTER agrees to take the dog to a licensed veterinarian at least once a year for an examination and routine vaccinations and tests: rabies, distemper, parvo, corona combination, provide monthly heartworm and flea/tick prevention year-round and provide the dog with veterinary care upon illness, injury or disease.
    8. FOSTER/ADOPTER agrees that the dog will reside in his/her home and will be kept as a household pet and will not be used as a guard dog or for breeding. The dog will not be chained, tied, or tethered.
    9. FOSTER/ADOPTER agrees the adoption of your dog from Golden Rescue South Florida is not final until you provide confirmation of implantation and/or registration of his/her microchip.   If you prefer, GRSF will enroll the microchip with lifetime registration with ACK Reunite.   Registration will be in your name with GRSF as a back up.   Confirmation can be emailed to: deniselibby@ymail.com or mailed to GRSF.
