    EMPLOYEE ACKNOWLEDGEMENT

  • I acknowledge that I have received, read, and understand HMT’s Code of Conduct. I understand and acknowledge that my conduct as an Employee of the Company must always comply with the standards and policies set out in the Code of Conduct, as well as any other legal or compliance policies or procedures of the Company. Other than anything identified below or that I know has previously been fully and accurately reported to the CEO, Legal or Compliance department, I am not aware of any apparent violations committed by other Employees in the course and scope of their employment and/or any third parties in the representation of the Company during the past twelve months. I will also report any violations that I become aware of in the future.

  • Once you have reviewed the Code of Conduct, please confirm that you abide by the HMT Code of Conduct, and you do not have direct knowledge of any apparent violations committed by other Employees or agents representing HMT that which has not already been accurately and fully disclosed to the CEO or the Compliance department. If you know of possible violations that occurred during the past year, either by yourself, by other Employees or agents representing HMT, you are required to disclose them. Please disclose them on this form.
