Please complete the PADI - Medical questionnaire by answering EVERY question on pages 1 and 2. Depending on your answers on the PADI - Medical form, you may need to have a doctors signature on page 3 at the request of IFS. If you answer "yes" to questions 3, 5, or 10 on page 1, OR "yes" to any question on page 2, you will need a Doctor's signature to be able to dive at Forfar.
