To nominate an adult volunteer for one of the GSUSA Adult Recognition Awards or Girl Scouts - Diamonds Adult Recognition Awards, please complete this Nomination Form and submit the appropriate number of Endorsers. Each endorser listed will recieve an email with the Endorsement Form. Submit one complete Nomination Form per person.

Both the Nomination and Endorsement Forms must be received no later than April 15 of each membership year. Online submissions will be accepted until 11:59 pm on April 15.
