Please fill this form out as the first step in registering for Advanced Manners. Submitting this form DOES NOT guarantee you a spot in the class. This form is to ensure your dog is a good fit for the group class! We want you and your dog to have a fun and enjoyable learning experience.
I understand that unless otherwise stated, group classes are not appropriate for dogs who growl aggressively, snap, attempt to bit, hide from, or show anxiety around other dogs or people. A dog exhibiting will be excused from a group class and given alternatives for training.
