Boating/Activties/MMA and Sport Waiver Release Form

Further, I hereby acknowledge and declare that:

 

 

In consideration for being allowed to use facilities, equipment, property, cabins, sand beach, mat room and everything else on the property of Friendship Island Resort LLC, and for other good and valuable consideration, the sufficiency of which is hereby
acknowledged:

I understand that during my stay there is no lifeguard on duty nor any medical proffesional personel, I shall be responsible for myself, all my guests and all my relatives using any and all equipment and/or facilities and being on Friendship Island Resort LLC property;

I understand that there are risks involved in stayingand or being present at the resort, using and/or spectating any and all activities taking place at Friendship Island Resort LLC (including any and all sporting events and or water related activities) such as severe injury or death.

 

 

Further, I hereby acknowledge and declare that:

 

I agree to indemnify, release, and hold harmless Friendship Island Resort LLC and its owner, Justin Hutter, and his family living on the property from any cost, liability, claim, damages, demand arising from injury, loss, illness, or death to me, my immediate family, guests, invitees related to the use of Friendship Island Resort LLC equipment, being on the property, using facilities and/or spectating events, use of the lake, use of property facilities, equipment and events (including any sporting event). Likewise, any damage or loss to personal property caused by or related to the use of Friendship Island Resort LLC equipment, being on the property, using facilities and/or spectating events.

Further, I hereby indemnify, release, and hold harmless the Friendship Island Resort LLC, Justin Hutter (Resort Owner), all his family and friends that live with him on Friendship Island Resort LLC from any claims, demands, damages, and costs arising out of negligence resulting to loss, injuries, illnesses, or death.

Finally, I recognize and fully understand and acknowledge the information above. By signing this form, I expressly declare that I am of legal age and that I am aware of the full contents of this waiver, and I am legally accountable to any effects of this Release.
