This questionnaire is designed to help Aloha Animal Oasis (AAO) and the Hawaiʻi Animal Kuleana Alliance (HAKA) find the best possible homes for adoptable cats and kittens. All adopting parties must complete this form before adoption and placement.

AAO and HAKA reserve the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions.

All cats are non-transferable. All applications are evaluated without bias. We evaluate based on the needs of the cat. Applications are not a guarantee of acceptance.
