WAIVER AND RELEASE FORM
Family Sports Center & Cheer Faces All-Stars LLC Program Waiver and Release form 2023-2024 As the legal guardian, or adult entrusted to care, of a gym participant(s), I recognize that potentially severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, including but not limited to gymnastics, tumbling, trampoline, rock climbing and stunting. Being fully aware of these dangers, I consent to the aforementioned person participating in any and all Family Sports Center and Cheer Faces All-Stars LLC activities and I ACCEPT ALL RISKS associated with that participation. In consideration for allowing my child to use these facilities, I, on my own behalf and the behalf of my child and your respective heirs, administrators, executors and successors, hereby COVENANT NOT TO SUE and FOREVER RELEASE Amanda Comer DBA Cheer Faces, Family Sports Center, it's officers, directors, shareholders, employees, coaches family and/or agents. Inthe event of an accident or emergency. I would like the below mentioned child to receive the appropriate emergency medical care, including hospital care if necessary, and I hold Family Sports Center & Cheer Faces AllStars LLC, Unity All-Stars, Faith Gymnastics and its representatives harmless in their execution of this action. Additionally, I hereby agree to individually provide for all possible future medical expenses that may be incurred by my child as a result of any injury sustained while participating at Cheer Faces/Family Sports Center or any company within, Including, but not limited to, Unity All-Stars and Faith Gymnastics, Karate, House of Pain etc.I acknowledge that potentially severe injuries may occur in any activity involving height or motion, and it is the express intent of the owner and/or staff to provide for the safety and protection of its participants. Should sickness or injury occur, I hereby give my permission for trained medical professionals to be notified Immediately and to administer emergency medical treatment, if deemed necessary, to the above named student. In consideration for permitting the above named student to participate in activities, I hereby release the owner and staff from all liability and for any and all damages resulting from injuries suffered by the above named student while under the instruction, supervision of said owner and staff. COVID-19 Liability Waiver: I acknowledge the contagious natureof thecoronavirus/COVID-19 and that the CDC still recommends practicing social distancing. I further acknowledge that Family Sports Center and Cheer Faces AllStars LLC and Unity All-Stars can not guarantee that will not become infected with the Coronavirus I hereby release and agree to hold Family Sports Center and Cheer Faces All-Stars LLC harmless from and waive on behalf of myself, my heirs, and any personal representatives, staff and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the gym.
