SMHS Activity Request Form

This form MUST be submitted online via Jotform at least 10 school days prior to the activity. Saturdays and Sundays do not count as school days. Once your form is submitted, you should receive a confirmation email from Mrs. Cervantes/Mr. Hong that your event is approved. In general, please leave the place cleaner than you found it. Thank you! (Note: We no longer loan out extension cords or power strips anymore.)
