I'm excited to see your interest in having a color experience with me! I've created this color consultation form as a quick way to analyze your current hair and your hair goals. It will ensure that the service block you choose or have chosen is the right fit for you and your hair needs.

After completing this form, return to the new guest page and complete steps 3 & 4.

I’ll review and compare your submitted form vs. the appointment block that you book online. If I feel that the appointment block needs to be changed or adjusted, I’ll contact you within 24 hours of our regular business days.
