PRE-EVENT FORM ... ONE FORM PER DOG!
This form is to be used for the {entry_type} entry of a single dog for the {hostclub} FastCAT test in Oahu. Each dog can sign up for one or two runs per day, space permitting.

It is essential that the correct Dog Owner/Handler information is entered including the valid AKC registration number for the dog and a valid email address. It is YOUR responsibility that all information entered is correct for any points to be awarded. --- CHECK IT !

A link to signup for time blocks of entered dogs will be provided at the end of this form. Please enter all dogs before signing up for time blocks. Any dog reserving a time block without being entered will be removed from the block without notice.

Note: This form is not to be used for "Fun Runs" or "Puppy Runs"
