I understand the risk of indoor tanning to my skin and overall health. Wearing protective eyewear is a requirement.
I understand that the employees here are trained and skilled to do their tasks and I should comply with the Tanning Salons' policy and procedure. The company will require me to wear protective eyeware during the session.
I will adhere to Sunshine & Tan Lines procedures for maintaining cleanliness and safety of the tanning bed and salon. I will follow the procedures for cleaning before and after each visit. I will follow procedures for entering and exiting the salon to ensure my safety and the safety of other members.
I understand that i am responsible for all properties within the tanning room during my visit. Should anything such as lotions, towels, or decorative items come up missing during my tanning session, or any damage to any rooms or beds or property of Sunshine & Tanlines, my credit/debit card on file will be charged for such items. I understand that Security Cameras located on the outside of each tanning room record my entering and leaving the tanning rooms and can be used against me if theft is suspected.
I understand that this membership is for one person only (the signer of this document) and that the membership is not to be shared with anyone else. I understand that letting someone into the tanning room without a membership is theft of services and against the law and I will be held accountable by the proper authorities should i be caught violating this law. No one is permitted in the tanning room unless they have signed up for a membership and completed this document.
I understand that failure to comply with any policies and procedures will terminate my membership.
I hereby absolve the tanning salon of any liabilities, claims, any lawsuit from my side for I was forewarned of the risk of indoor tanning to my skin and overall health.
The company also affirmed the harmful effects of overexposure to ultraviolet radiation emitted by the tanning device may cause skin burns, premature skin aging, melanoma, and skin cancer.
I understand that a valid credit/debit card must be kept on file to keep my membership, if a card is removed from the payment source your membership will be terminated immediately
I understand the payments for my membership are automatically withdrawn on the same day each month according to the date i signed up and if payments are not completed the date they are due my membership will be terminated the following day
Should your membership have to be cancelled due to non payment or removal of payment source you are subject to be banned from the tanning service permanently
To cancel or pause your membership you must notify us by text message a minimum of 5 days prior to your next payment, no refunds will be given for last minute (day of, day before) notifications.
