ISTT Committee Application Form

This form must be entered by the applicant. The applicant should inform one's Society about the application prior to submission.

At the end of this form, the applicant will be asked to upload a file (PDF) that contains the following information:

1) One-page bio sketch
2) Photo-head shot preferred
3) Listing of years of ISTT membership with Affiliated Society name, category of membership (i.e., as an individual or under a corporate membership)
4) Corporate membership name, if applicable
5) Listing of ISTT International No-Dig Conferences attended as a full registered participant
6) Listing by reference title for the papers published in ISTT Conference Proceedings.
7) Listing of the years of services and position with the Affiliated Society
8) Explanation of reasons why the criteria above cannot be achieved, if applicable.
